Eligibility Criteria
Essential Qualification: Students should have passed HSC Exam/Intermediate (10+2) or its equivalent exam with Physics, Chemistry, Biology and English Securing pass with min of 45% marks in aggregate from recognized Bord. 5% relaxation allowed to reserved category having domicile of Uttar Pradesh.
Age: Min age for admission to BSc course shall be 17 years and maximum age 25 years as on 20 June of beginning of the season. Age relaxation to reserved category will be as per UP State Govt rules and domicile of UP only
Application Fee:
Students will be required to pay sum of Rs.1000/- as application fees online before filling up the form to Director, SGPGIMS Lucknow Academic Account, Payable at SBI, SGPGI Branch (code no.7789), Lucknow which is not refundable
How to Apply
The application form along with information brochure can be downloaded from the institute website www.sgpgi.ac.in OR 5.9.212.47/pgi_perfusiontechnology during 25 July to 10 August 2018. The candidate should read the instructions very carefully before filling up the form. The candidate will have to fill up the Application Form oNline. After online submission, printout of filled in application form must be taken and send to Deputy Registrar, College of Medical Technology, SGPGIMS, Rae Bareli Road, Lucknow-226014 along with all relevant documents in support of age qualification, experience, reservation and sponsorship etc and duly signed by the candidateThe last date of receiving completed application forms along with necessary enclosures is 15th August, 2018. Candidates will be accepted only of those who will apply through online and also send a hard copy at the above mentioned address of the Institute
