Registration Form

Part-I Registration

1) Select Payment Mode for Registration
a) Online (Debit card/Credit card/Internet Banking/UPI)
b) E-Mitra Kiosk
2) If you select online mode payment it will be directed to Rajasthan Part-I screen Follow steps 4 to 16
3) If you select e-Mitra Kisok mode then go to any e-Mitra Kisok. Select the Service the service name as e-Mitra e-Mitra sso>Services>Avail Service> Utility Services search> Medical Education-MBBS/BDS admission 2018
4) Provide your NEET Roll Number
5) Your Name as filed in NEET 2018 forms appear on screen
6) Enter name of Father
7) Select Date of Birth (as it appears in your 10th class mark sheet)- Day, Month and year
8) Select your community category (General/SC/ST/STA/OBC-CL/OBC-NCL/MBC-CL/MBC-NCL) and give certificate date of issue for OBC NCL MBC NCL
9) Select PH category (if applicable)
10) Select Defence/Para-miliatray (if appclaible)
11) Write your NEET 2018 marks
12) Select percentage of marks obtained in 10+2 or an equivalent exam of the subjects together Physics, chemistry, biology/biotechnology
13) Enter email ID (if you don’t have create a new email ID or use your relative email id)
14) Select Domicile state
15) Select your family income is less than INT 2.5 lacs (if applicable)
16) Select other eligibility criteria (regard continuously resided in Rajasthan, bonafide residents of Rajasthan, candidate is son/daughter of serving or retired govt Organisation/university,
17) Press “Proceed”
18) Check entries made in preview, choose edit if any fields modify then submit
19) Make the registration fee payment. Note down your registration ID and password carefully

Registration Fee
As per Honable Supreme Court order dated 9 May 2017, the candidates admitted to all medical / dental colleges will have to deposit fees only through Demand Draft payable to the allotted college (refer table available at website – “DD in favour of”) for one year at time of reporting in Round 1 and Round 2 counselling at SMS Medical college, jaipur to the State Counselling Board which will be transferred to allotted college. Part-2 Registration

Candidates after depositing the registration fee, login the registration part 2 by giving registration id and password Basic Details
1) Select appropriate salutation (Mr/Miss/Mrs/Ms
2) Name will appear on screen (it not editable)
3) Date of Birth also shows it non editable
4) Select state of birth
5) Select gender: Male/Female
6) Write mother;sname
7) Present communication address (full present address along with city, district, state and pincode
8) Permanent address (if both present and permanent address are same, choose above address or differ write new address ie it may called native address
9) Write your STD code for resident phone number, it don’t have leave blank
10) Write mobile/cell number without Zero ie only 10 digit number
11) Your email ID as displayed on screen
12) Your domicile state is exhibited on screen
13) Select Nationality: Indian/Other
14) In case other write address and other contact details of parents and address of local guardian

Educational Details
A) Secondary (10th ) or equivalent examination (write board name, year of passing, percentage of marks)
B) 10 + 2 or equivalent examination(write board name, year of passing, select no of attempts for passing, percentage of marks)
C) Enter name and place of college / school last attended.
D) Select course you wish to apply: MBBS only / BDS only / Both MBBS and BDS
E) Press Save and Next

Documents Uploading
Choose and upload the applicable documents (photograph, signature, caste certificate (only for ST-STA), ph certificate (if applicable) one by one and check change in the status. You uploaded images appear on screen after verifying showing correctly press Save and Next


Choice Selection / Filling

1) To Fill Choices, select the course and college combination in order of your preference
2) You must choose min 1 choice / option. You may choose as many choices/options as you wish. There is no restriction for filling max number of choices
3) Now press Add Choice button
4) Verify all entries if OK click Save button
5) It is essential to save your choices by pressing the save button. If you don’t save choices will not save, you may click reset button selecting choice freshly
6) Press Logout button. Message will show at time of logout. Are you sure, you have saved your choices by pressing the save?

Modify / Delete Filled Choices
You may change your order of preference by deleting choices and then adding the fresh choices. You can use this option as many times as you wish before clicking save and next. You can also move/toggle/delete any choice. For this select particular save newly added button after each modification. You can also remove newly added choice by clicking “Reset newly added” . You can also remove all choices by clicking ‘Reset all choices” Click save and next.

Declaration
Declaration will appear on screen. Press “I Accept” on this declaration it is must read and accept, Now here view preview the relevant details you entered “Check and Verify” once you decided all are correct press Final Submit. After final submit also you can edit your details in Part-2 and choice filled till 27.06.2018 (11.45PM)

Documents Required For Counselling

After registration completed by completing above part-i and part-ii registration in the process choosing choice filling modify choices take the printout two copies that form and keep the following documents compulsory with you, The board will ask you at the time of counselling
1. NEET 2018 Score Card
2. Allotment Letter
3. Mark sheet of 10+2 (senior school certificate examination) or its equivalent
4. Date of Birth Certificate: 10th class mark sheet / certificate or any other equivalent certificate showing the date of birth
5. Mark sheet of secondary examination (10th class)
6. All mark sheets of Class 11 and Class 12 if passed in more than one attempt
7. Certificate/ Document providing your eligibility as selected by you at the time of filling on-line registration form- (I to XI) - as given under serial 3 in the information booklet
8. Copy of photo ID (driving licence / PAN card / Voter ID / Govt. or PSU card / School ID card / 12th class examination admit card / Aadhaar card)
9. Valid Caste certificate, sub-category certificate, if applicable
10. Domicile certificate, if applicable
11. 4 Passport size photo same as affixed on registration form
12. PH certificate, if applicable
13. WDP / WPP certificates, if applicable
14. Bond as per the applicable Proforma
15. Any other relevant certificate(s)

Selection of Students
The selection of students to a medical/dental college shall be based solely on the All India merit of candidates through NEET 2018. To be eligible for the Rajasthan MBBS & BDS admission 2018, the candidate must have fulfilled the eligibility criteria as mentioned in clause 3. the eligible qualified candidates in NEET 2018 will be admitted in the order of merit for the general as well as reserved seats expect the setas belonging to category of sub clause d of clause 1

Allotment of Seats

Allotment of seats through Online allotment process of all NEET qualified and registered candidates through NEET 2018 result shall be conducted for all candidates who appeared in NEET 2018 strictly in accordance with merit list prepared on basis of All India Merit in NEET 2018, the order of choices / options filled by candidate in oneline registration form
